After 8 years of home health care experience, we have determined that a patient's mental and physical condition improves more rapidly in the comfort of the home setting. Divine HealthCare brings comprehensive health care services into the privacy of the patient's home. Not only does Divine's in-home services contribute to the patient's quality of life but it also helps prevent costly and inappropriate hospital stays. Highly skilled professionals, under a physician's supervision, can provide the following homecare nursing services:
For nearly 8 years, Divine HealthCare Network has been a voice in Homecare, providing patients with trusted, dependable and experienced caregivers in the Twin Cities Healthcare field.
We provide a variety of home health care services to meet your individual needs. From skilled cares to personal assistance, our services are available 24 hours a day, 7 days a week. At Divine HealthCare, we operate with a family-centered approach. We believe that your family is an important part of the Divine HealthCare team and your recovery. We do our best to make sure you and your family members understand and feel comfortable with all services we provide. We also believe we have a responsibility to provide the best and most experienced staff.
We accomplish this through our hiring process. All applicants are carefully screened for training and experience that meets the needs of our patients, references are checked, and licenses and certificates are verified and background studies are performed. We have a diverse pool of home care personnel to meet our clients' ethnic and cultural preferences. of our clients. We provide home health care services mainly in the seven county metro areas, but also in the greater Minnesota locations.
Employment
We employ Registered Nurses (RN), Licensed Practical Nurses (LPN), Home Health Aides (HHA), Personal Care Assistants (PCA), Homemakers, Live-in-Aides, Physical Therapists (PT), Occupational Therapists (OT), Speech Therapists (ST), and Medical Social Worker.
Assessment
Initial evaluations and assessments to begin services and case management are done by Registered Nurses. “Therapy only†cases are routinely managed by a therapist, but first assessed by a Registered Nurse. Divine HealthCare utilizes a case management system to provide comprehensive coordinated care and services to clients served in the Twin Cities
Divine HealthCare is a Medicare Certified, Class A Licensed home care provider. In addition, Divine HealthCare is enrolled as a personal care and waivered services provider with Minnesota Health Care Programs. We also have County contracts with Hennepin, Ramsey, Anoka and Washington counties and other counties on a request basis. Divine HealthCare is contracted to provide home care services for various insurance companies such as Medica, Health Partners, U-Care, Blue Cross/Blue Shield, United Health Care and Tri-West.
We accept Medicare and Medicaid clients, HMO’s or private insurance and private paid individuals.
Our Goals:
Divine HealthCare holds that Homecare services are vital in the National Health Care Delivery System. We provide continuing care to clients in the comfort of their own homes thus reducing the stress, discomfort, and anxiety that would otherwise add to illness and pain.
To achieve this purpose, our goals are to:
-Provide all levels of home health care services under the directives and guidelines of the Human Service Department.
-Provide home Healthcare services in collaboration with clients’ physician in providing effective care and services.
-Provide cost-effective competitive and quality care services by effectively matching client needs with skilled personnel.
- Develop relationships with referral sources and Community waiver Services in order to effectively meet client’s care needs in the Community.
- Utilize qualified personnel, well experienced in Home Healthcare services.
