We currently have an exciting opportunity for a Director Disease and Case Management to provide strategic direction and leadership in the development and execution of innovative, industry leading member/patient-centered strategies that ensure high performing, successful disease and case management programs for HealthPartners health system.

Accountabilities:
1. Develops and directs the short and long-range strategic and tactical plans designed to meet the objectives of disease and case management programs as measured by the triple aim (experience, health, affordability) and in response to market trends.
2. Directs lean operational functions and process improvements within the program, including effective and efficient integration with other areas within the division and across the organization.
3. Directs structured process to ensure automated systems are maximized for productivity and outcomes; maintains current industry knowledge of innovative applications that enhance proficiency and effectiveness.
4. Directs development and maintenance of policies and procedures to ensure compliance with regulatory requirements, accreditation standards, and organization expectations.
5. Responsible for the development, execution, monitoring, and maintenance of medical management vendor contracts including routine, timely payment authorization and maximization of contracted services.
6. Directs responses to RFP/RFI requests, employer reporting, performance guarantees, and consultation in partnership with key stakeholders in the process.
7. Represents HealthPartners purchaser, audit, regulatory and other meetings and forums, independently and/or in conjunction with other senior leaders.
8. Responsible for the development and management of the department’s capital and operating budget ensuring effective use of all resources to achieve successful program outcomes and positive ROIS.
9. Provides leadership and represents Disease and Case Management in task forces, committees, projects, and other groups that may be interdepartmental, organizational, or external to the organization.
10. Directs and participates in the creation and maintenance of business continuity and disaster recovery planning.
11. Partners with other Disease and Case Management department leaders in working effectively with other functional areas in a matrix management situation.
12. Responsible to develop and mentor a high-performing management team leading to high-performing front-line staff
13. Develops, manages, and nurtures strong linkages and relationships with internal and external customers, partners, and key stakeholders that are vital to the successful operation of the department, division, and organization.
14. Participates in ongoing independent study, education related professional activities, and professional affiliations to grow knowledge and expertise in relevant fields of practice.
15. Performs other duties as assigned.

REQUIRED QUALIFICATIONS
• Bachelor’s degree in business or related health care field.
• Relevant certification coursework within 24 months of employment.
• Demonstrated effective, independent leadership and management skills in the development of new and ongoing programs that have resulted in measurable improvement in experience, outcomes, and affordability.
• Demonstrated skills in program management, project management, time management and/or resource management.
• Demonstrated experience and success in working independently with physicians and in senior management level activities.
• Demonstrated flexibility, organization, and appropriate decision-making in challenging situations.
• Demonstrated ability to observe, identify and effectively communicate key medical management trends that will impact business development and operations of the department in the short- and long-term.
• Expertise with large budget management, pricing of services, contract management, and ROI projections.
• Demonstrated ability to influence change.
• Strong skills in team building and collaborative relationships.
• Proven oral and written communication skills.
• Proven ability to effectively use personal computer software applications such as MS Word, Excel and PowerPoint.

PREFERRED QUALIFICATIONS:
• Master’s degree in business, health care administration or currently enrollment in a master’s program.
• Experience in the delivery of multiple dynamic program services to large populations.
