You will provide case management and skilled nursing services to clients in their homes. Primary job duties will include: coordinate care; participate in interdisciplinary team; oversee careplan; provide direction and supervision to Home Health Aides and PCAs.
This FT position works Monday through Friday with lots of flexibility and autonomy! You will have an assigned client caseload in a designated travel area with the Twin Cities metro. You will be required to travel to the company office only 2 times per week. Company will provide a paid gas card. Great training is available and new grads are welcome. Previous experience in homecare a plus.
