Mercy Hospital, located in Coon Rapids, Minn. serves the northern metropolitan communities with a full continuum of care and services and has been nationally recognized for its expertise and outcomes in cardiovascular care. With 2,300 employees and 239 staffed beds, Mercy provides inpatient and outpatient services to nearly 140,000 patients each year. It shares its nearly 842-physician medical staff with Unity Hospital in Fridley, Minn.


Responsibilities

  • Coordinate discharge planning to occur on the weekends and assure plan of care progression
  • Includes participation in level of care monitoring and assignment
  • Meeting regulatory guidelines and requirements
  • Completing discharge appeals and addressing and overcoming barriers to appropriate discharge for patients
  • Includes utilization management functions in communication and negotiating with insurance companies
  • Implement and track readmissions patient issues and solutions

Qualifications

Education & Experience:

  • Registered Nurse with a BS or BA (required)
  • 3 Years of recent acute care experience in medical/surgical clinical areas with proven strong experience in collaboration with interdisciplinary teams
  • RN License

Skills:

  • Demonstrates leadership experience and ability to work independently
  • Excellent communication skills with the multidisciplinary care team to include problem solving and critical thinking in an autonomous work environment
  • Ability to build and leverage relationships in order to progress the plan of care and promote timely discharges
  • Detail orientated and flexible to navigate multiple priorities

Pay,Benefits, & Work Schedule

  • Part time (Day shift)
  • 7-1530 Every Friday, Saturday and Sunday
  • Alternative weekend plan
  • MNA  Contract

The city of Coon Rapids is a fast growing community on the Mississippi River just 20 minutes north of Minneapolis-St. Paul, and the metropolitan area's gateway to the northern part of the state. Coon Rapids offers excellent access to highways, dynamic community shopping centers with major retail anchors, and a wide variety of recreational activities including neighborhood and regional parks, a municipal golf course, indoor ice arenas, hiking and biking trails and swimming pools.
