Allina Home and Community Services (AHCS) is the trusted provider and expert resource in delivering patient-centered care and services for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 19 Minnesota counties, 24 hours a day, seven days a week.


  • Nursing assessments/visit to patients in their homes, nursing homes, assisted living facilities
  • Pain and symptom management of the dying patient
  • Care provided in collaboration with interdisciplinary team


Education & Experience:

  • RN License MN
  • BSN Preferred
  • BLS Required
  • Minimum 3 years critical care experience
  • Oncology background with strong knowledge of pain management background
  • Experience in hospice/homecare preferred


  • Strong written and verbal communication skills
  • It requires strong assessment skills and a proactive approach to planning care in the home
  • Critical thinking and problem solving
  • Able to work independently and autonomously
  • Sensitivity and skill working with patients and families in crisis
  • Ability to utilize computer for documentation

Pay,Benefits, & Work Schedule

  • Casual position (No guarantee of hours per week)
  • Day shift
  • 1 weekend per every 4 weeks and casual per pt needs
  • Non contract
