Allina Home and Community Services (AHCS) is the trusted provider and expert resource in delivering patient-centered care and services for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 19 Minnesota counties, 24 hours a day, seven days a week.
Responsibilities
- Pain and symptom management decisions
- Appropriateness of patients for hospice per Medicare and non-cancer diagnosis guidelines
- Determination of level of care need
- Determination of patient service needs
Qualifications
Education/Experience:
- ADN/BSN
- Oncology or ICU helpful
- 1 year previous home care experience with 1 year hospice helpful
- 3-5 years of nursing experience required
Skills:
- Customer service knowledge and skills
- Knowledge of Medicare, MA and third party payor benefits
- Good communication and listening skills
- Ability to apply clinical nursing skills with emphasis on pain and symptom management, IV skills, care of dying patient, general physical assessment skills
- Empathetic/compassionate personality
- Problem solving/critical thinking ability
- Needs computer skills and ability to function within 2 or 3 different programs
Licensure/Certification:
- BLS
- Medical Terminology Required
- MN RN License
- Current Driver's License, Insurance
Pay,Benefits, & Work Schedule
- Full Time Position (40 Hours/Week)
- Day Shift
- Every 4th Weekend
- Non Contract
