Physical Therapist-Home Care

Responsible for planning and implementing physical therapy assessments and treatment procedures in the home setting with the client and their family while working within the scope of PT registration and under physicians order.


This position will require frequent travel throughout the SW Metro (i.e. Jordan, Chaska, Eden Prairie, Shakopee, Lakeville, Bloomington etc...) 


License/Certification requirements: Current Physical Therapy license in the State of Minnesota. Current CPR (BLS for Health Care Providers) certification.


Experience required: Minimum of one year recent experience as a staff RPT in a clinical setting.


Basic computer skills, reliable transportation, valid MN driver's license (or license that matches your current permanent residence), current and adequate auto insurance.


Preferred experience: Previous home care experience.
