Southview Acres Health Care Center, a busy metro facility with two TCUs, Memory Care and LTC is seeking:

· PM Building Charge (RN) ·

Sign-on bonus of up to $5000

Provide clinical and personnel support to 241-bed long-term, transitional and memory care facility. Strong clinical skills and supervisory experience required. Experience troubleshooting EMR and computer issues necessary. 3-5 years management experience preferred.

· Licensed Social Worker (LSW) ·

Qualified applicants must have at least one year experience, knowledge of discharge planning and a proven track record of paper compliance and patient care planning. Working knowledge of state regulations is preferred. Assists in providing services to meet the social and emotional needs of the residents/patients.

· Director of Therapeutic Recreation ·

Seeking a dynamic individual to lead TR. Best candidate has experience with an interdisciplinary team leading program development from medical model to social model. Strong resident-centered care experience required. Knowledge of regulations and documentation a must, volunteer recruitment and management is desired.
