Job description:
We are currently seeking a part time Family Nurse Practitioner to work for our Health Care for the Homeless (HCH) program in St. Paul.  HCH serves approximately 3,500 homeless people per year in 8 shelters and drop-in centers in St. Paul.  HCH addresses the housing, health care, addiction, and mental health obstacles that prevent homeless people from stabilizing their lives.  This position is 27 hours per week, and may include evenings and weekends. Come join our dedicated staff who help serve the underserved! As part of our staff, you will have the following responsibilities:

Primary Duties:

  1. Provide primary care delivery.  Utilize appropriate and effective patient care plans including adequate assessment, diagnosis, treatments, consultations and outcomes.
  2. Manage patients to assure continuity of care including adequate scheduling of return visits, completion of treatment plans and follow-up of chronic problems, tests and no-shows.
  3. Provide appropriate interpersonal care which includes introducing oneself; treating patients with care, compassion, and courtesy; responding in a non-judgmental and culturally sensitive manner; listening to and answering questions from patients; and clearly explaining diagnoses and treatments.
  4. Provide whole person care by using appropriate referrals to the social worker, developmental specialists, and by integrating the biological, social, psychological and environmental factors.
  5. Utilize cost-containment measures by prescribing only necessary medication, generic when possible, and ordering only necessary referrals, procedures and tests when appropriate.

Secondary Duties:

  1. Provides appropriate patient education including explanation of diagnosis, treatment, prognosis and use of medications.  Instructs patients about health care maintenance and using appropriate patient education handouts and classes.
  2. Provides and documents periodic health screening and preventive care, including baseline and interval physical exam, as well as recommended testing within standard of care guidelines.
  3. Interacts appropriately with staff members which includes treating staff with respect and courtesy and informing the front desk if more than 15 minutes late for clinic session.
  4. Demonstrates appropriate work habits, including arriving on time, returning phone calls, signing charts in a timely manner, participating in peer review and meetings.
  5. Assists in the ongoing improvement and development of services at WSCHS by contributing ideas to the Medical Director, participating in the health care teams, and by understanding and incorporating health care plan objectives into day-to-day practice.
  6. Maintains adequate documentation of patient care including problem records, medication records, drug allergies, subjective and objective data, assessments, treatments, patient education and follow-up.
  7. Assumes administrative responsibilities including the physician performance audit, quality teams, and assignments agreed upon by the clinician and Medical Director.
  8. Participates in professional networking activities, including clinician meetings, all staff meetings, teaching students and residents, and models the WSCHS community oriented primary care philosophy to staff and clients.
  9. Participates in community oriented activities including presentations in the community, attending open board functions and community celebrations as available.
  10. Demonstrates commitment to the mission and goals of WSCHS and serves as clinical role model and leader for other staff.
  11. Enhances patient education and focuses on primary prevention.

Qualifications:

  1. Completion of accredited nurse practitioner program.
  2. Active Minnesota license.
  3. Previous experience preferred.
  4. Skill in listening, counseling, interviewing and communicating to elicit information and cooperation from patients.
  5. Skill in establishing and maintaining effective working relationships with patients, staff and the community.
  6. Ability to evaluate, diagnose and treat patients.
  7. Ability to demonstrate good work habits and assist in the improvement of the services at WSCHS.
  8. Bilingual skills in English/Spanish preferred.
