MDS Registered Nurse
The Homestead at Anoka Inc
The primary purpose of the MDS Nurse is to plan and organize the MDS/RAI process. This position will transition to The Homestead at Anoka; the campus features 120 skilled nursing suites and 59 assisted and independent living apartment homes. Our residents will be living in home like neighborhoods with many amenities including club room, general store, and chapel/community rooms. We provide our residents with a home in a resident focused culture.
Qualifications:
- Current Minnesota RN license
- Two (2) years experience in a long term healthcare setting with RAI/MDS process
Essential Functions:
- Completes assessments, MDS, and care planning
- Work with IDT team to determine significant changes in resident status/care planning
