The On-sight Health and Wellness Program Manager is responsible for the day to day management of HealthPartners worksite programs including planning, development, implementation, evaluation and maintenance of health promotion, risk reduction, disease self management and demand management. This person will also be responsible for providing direct member health coaching.

ACCOUNTABILITIES:

1. Develop, coordinate, implement, and evaluate HealthPartners worksite activities.

2. Develop health promotion program plans that target populations based on analysis of claims and utilization, health risk and productivity assessments with appropriate objectives to achieve measurable outcomes.

3. Assist in the development of a health promotion program mission, goals, objectives, and identify outcome measurement strategies.

4. Develop detailed project plans, timetables, resource estimation and tracking processes to meet all deadlines.

5. Consult with clients to facilitate changes in organizational culture supportive of health improvement initiatives.

6. Consult with clients to identify appropriateness of various types of program plans and facilitate integration of health improvement into their organizations.

7. Apply the worksite program to optimize integration of communications, marketing, programming and interventions.

8. Facilitate cross-company project team focused on initiative implementation, as needed.

9. Provide direct member lifestyle/behavior change counseling on behavioral health issues, based on health risk appraisals or member inquiries. Core areas of focus include physical activity, tobacco cessation, stress management, and weight management, other health-related areas of preventive counseling.

10. Collaboratively develop a wellness plan with the employee and then follow-up with employees to help facilitate behavior change.

11. Help participants select health improvement programs, including HealthPartners health improvement programs, company programs, and community programs.

12. Facilitate ongoing communication between customers and HealthPartners to coordinate and optimize programming.

13. Identify and act upon barriers to success and work to achieve plausible solutions.

14. Perform other duties as required to accomplish HealthPartners and company goals, as assigned by the Worksite Wellness Coordinator Supervisor.

REQUIRED QUALIFICATIONS:

* Bachelor of Science or Bachelor of Arts in health related field.

* Three to five years of experience in a health promotion setting.

* Program development, implementation and evaluation experience.

* Knowledge of the principles, theories, and practices of worksite health promotion, risk reduction, disease self management and demand management.

* Project management skills.

* Proven ability to work collaboratively with strong problem-solving skills.

* Detail oriented with strong organizational skills.

* Strong written and verbal communication skills.

* Computer skills to support data analysis and reporting (Excel, PowerPoint, Microsoft Word and Visio)

* Ability to deal with ambiguity and changing priorities.

* Ability to work with all levels in the organization.

PREFERRED QUALIFICATIONS:

* Master’s degree in Nutrition, Exercise Physiology, Community Health Education, Public Health, or other health-related field.

* Previous experience in one-on-one counseling in a worksite setting.

* Worksite experience in an employer setting.
