Minnesota Masonic Home North Ridge has a rich history of providing quality care to seniors since 1966. We are a specialized senior health care community. Our continuum of care offers a variety of services: Senior Apartments, Assisted Living, Adult Day Program, Complex/Transitional Rehab Care, Long Term Care and secured areas for those with memory impairment. Our emphasis at MMH North Ridge is to recognize and provide for the needs of each individual, whether the goal is to remain in our community or to return home. The LPN position will be working in the Assisted Living and Apartment building. There are three Assisted Living areas in that building with a total capacity of 66. These areas offer four levels of care and service to individuals in need. Our residents are encouraged to maximize their independence while having care available when needed. The assisted living areas include all utilities, all meals, housekeeping, laundry and linens, one bath or shower per week, medication and lab management. Many additional supportive services are available as needed. Our Assisted Living is licensed by the State of Minnesota as a Housing with Services establishment. In addition, we have 128 independent Apartment units in which we offer a variety of amenities. Home care services are offered on an as needed basis to the apartment residents. The LPN-Assisted Living position is responsible for assisting the RN with the responsibility of total resident care for apartment and assisted living residents. The LPN performs professional nursing duties in accordance with agency policy and state home health regulations. Major job functions include: Monitoring the Home Health Aides (HHA)performance; setting up and monitoring medications of residents as ordered by MD; performing treatments as ordered by physician; assisting in developing plan of care from service agreement and revise as needed; assigning HHA according to plan of care and assists with assignments as needed; assisting in orientation of new employees; notifies MD of resident changes and concerns after consulating with RN; receives and processes MD orders; attending family conferences; exercising confidentiality to ensure compliance with all HIPAA regulations, etc.


The successful candidate must be a graduate of a practical nursing program for LPN and have current licensure with the MN Board of Nursing. Position requires CPR certification. Prefer six months experience in medical, home health or LTC nursing. Requires good understanding of medical, social and psychological needs of the elderly and rehab principles. Must have good communication skills and be able to ensure confidentiality of all resident care needs. This position is working 0.4 PM (3:30p-9:00p) and includes every other weekend and a holiday rotation. Total hours per pay period are 22 hours.
