Mary T. Inc. Home Health has an opening for an LPN to be the Intake Coordinator. This is a full-time position that will office out of Coon Rapids. Duties include:
- Completion of documentation for new referrals
- Interviewing of patients or representatives to obtain insurance and personal information and to identify the appropriate payment source
- Assignment of new admissions to a skilled nurse
- Maintenance of patient records in the Carefacts software system
Requirements include:
- Minnesota LPN licensure
- Some knowledge of intake and admissions policies for Home Health patients
- Familiarity with insurance requirements for Home Health
- Experience with maintenance of electronic patient records using strong computer skills
- Excellent comminication and public relations skills
- Organization skills with a strong attention to details
