Position Summary

The Care Management Coordinator is a member of the healthcare/treatment team. The CMC collaborates with the team to coordinate care management services that are designed to ensure access and to follow the care of patients across the continuum of care. The CMC works to promote and assure service accessibility, attention to individual needs, continuity of care, comprehensive and coordinated service delivery, culturally appropriate care and fiscal and professional accountability and on-going quality improvement. Bachelor's degree or equivalent health related education and experience. Previous clinical quality improvement, utilization review, discharge planning or case management preferred. Must have a valid driver’s license and meet the insurability requirements relating to Park Nicollet’s motor vehicle violation criteria. Additional Position Summary Detail: 2+ years Eating Disorder, Mental Health, coaching, or social experience and leadership experience preferred. Skills and Abilities required: a.) Ability to use critical thinking skills b.) Possesses ability to work collaboratively and establish rapport with manager and staff of multiple disciplines to assure positive outcomes. c.) Assertive interpersonal behaviors d.) Establishes person and professional credibility in the organization e.) Mature verbal and written communication and interpersonal skills f.) Demonstrates strength in managing complex situations, teaching, functioning as a positive change agent, conflict management, systems thinking and situation analysis. g.) Commit to creating a positive patient experience h.) Willing to act as a patient advocate  This position provides psychotherapy to patients and requires Park Nicollet to comply with the Minnesota Statute 148A.
