PHN to conduct assessments in the St. Cloud & Willmar areas. Part Time. Must have BSN degree & PHN certificate. In-home health care experience a plus. Complete assessments, attend meetings, heavy travel. St. Cloud/Eagan office - very flexible, comp salary (pay per assessment).
