The Care Coordinator - RN provides a deliberate patient and family centered partnership with the healthcare team that facilitates a seamless patient experience between the Inpatient and Outpatient settings.  The Care Coordinator works to maximize the patients health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits.  Works to develop and support a coordinated plan of care between visits to promote a smooth transition for patients and family across the healthcare continuum that instills a sense of trust and patient advocacy.

In order to qualify for this role you must possess:

  • Current MN RN Licensure and knowledge of specialty area
  • BLS certification
  • 3 years of clinical nursing experience, preferably within specific patient population group
  • Proven ability to function at highest level within scope of practice
  • Patient education and pro-active disease management experience
  • Excellent assessment and care coordination skills
  • Experience with an EMR
  • Keyboarding/computer skills
  • Strong customer focus to accommodate patients
  • Proven ability to follow protocol for specific care delivery expectations per sub specialty population


  • BSN or MSN
  • Specialty certification or training
  • Knowledge of case management
  • Patient advocacy experience
  • Patient population specific certification
  • Previous inpatient care experience
  • Previous experience providing patient family centered care and care coordination across the healthcare continuum
  • Excellent time management, prioritization, multi-tasking and organizational skills
  • Ability to work in multiple computer systems
  • Skilled in using EPIC
  • Effective coaching skills
  • Critical thinking skills
  • Ability to provide lifestyle and chronic disease/complex case management services
  • Due to differences in patient population needs, the specific experience required for this position may vary as noted in Addendum
  • Ability to cross cover for peers as needed
  • Ability to build positive relationships
  • Ability to delegate appropriate tasks while functioning within a beam based model
  • Flexibility, adaptability and teamwork
  • Motivational interviewing skills
