Clinical Case Manager
This position provides a broad range of clinical case management services within 5 specific, pre-designed programs. This position functions with a high degree of independent accountability, in accord with professional and ethical standards and organizational policies.
The Clinical Case Manager is responsible for managing a caseload of clients dealing with both mental health and chemical dependency issues. The Clinical Case Manager acts as liaison between various levels of inpatient and outpatient care facilities and the client. He or she assists clients with the development of social and community support systems that promote stability and enable the client to function at the highest level possible. Clinical Case Manager offers his/her extensive knowledge of community resources and support as a primary means of assisting each client to reach their rehabilitation and recovery goals.
- Master’s degree in social work, counseling, clinical psychology or related field.
- LICSW, LGSW, LADC and 1-2 years experience preferred.
- Bachelor’s degree with 3 years plus experience may be considered.
Essential Duties and Responsibilities
- Provides clinical case management services to program participants through a short-term case management model.
- Maintains regular contact with program participants, clients and local support resources as outlined by participants’ prescribed continuing care plan.
- Provides advocacy, crisis intervention, counseling and problem solving in a variety of areas for all program participants, when appropriate.
- Conducts outreach and intake as requested.
- Provides program participants with support in order to increase or restore program participant’s capacity for self-sufficiency.
- Makes referrals in accordance with service plans to other organization departments and external providers.
- Develops, implements, oversees and follows up with program participant treatment plans to ensure that realistic goals and deadlines of accomplishments are being met. Submits written reports to participant’s collateral contacts that outlines their progress.
- Accurately completes in a timely manner all necessary forms, case recordings and statistical reports.
- Develops, monitors and enhances professional relationships with social service, health and governmental agencies.
- Participates and contributes in regular supervision meetings, team meetings, in house training sessions and conferences.
- Participates in program and operational planning.
- Participates in periodic evaluative reviews and/or in-house and external staff training to ensure that he/she understands and continues to adhere to clinical standards. Initiates requests for assistance from Program Director to address new issues or complex concepts affecting adherence to professional standards.
- Maintains an on-going caseload while successfully functioning as a team member.
- Ability to read and interpret documents.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of customers or employees of organization.
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
- Working knowledge of Microsoft Word and similar word processing programs.
