Allina Home and Community Services (AHCS) is the trusted provider and expert resource in delivering patient-centered care and services for patients and families needing rehabilitation, advanced-illness or end-of-life care. With headquarters located in St. Paul, Minn., services are provided in 19 Minnesota counties, 24 hours a day, seven days a week.


Responsibilities

  • Provide palliative care consultation at United and St. Francis hospitals
  • Intervene and make recommendations for pain and other distressing symptoms
  • Facilitate goals of care discussion
  • Facilitate family members

Qualifications

Education / Experience:

  • Masters in Advanced Practice Nursing
  • Minimum 5 years clinical experience, with preferred experience in palliative, oncology, geriatric, and/or hospice care
  • Advanced practice nursing license required
  • BLS required

Skills:

  • Excellent communication skills - verbal and written
  • Highly organized, self-directed and motivated
  • Creative and innovative problem solver

Pay,Benefits, & Work Schedule

  • Part time (20 hours/week)
  • Shift: Days, Monday - Friday
  • Non-contract
