Licensed Practical Nurse for a Senior Assisted Living Facility
LPN is responsible for the direct supervision of care giving within the residence.
Resident Care:
Manages the health care of all residents, including the dissemination of information to families and staff.
Facilitates continuity of care for those residents receiving services.
Communicates with resident’s physicians re: resident’s status and condition changes.
Assesses residents on a scheduled and as-needed basis, records significant conditions and changes.
Notifies Administrator/physician/family of resident’s condition and changes, treatments and significant incidents.
MN unrestricted LPN license
2 years geriatric experience
Monday through Friday 12pm to 8pm
