Allina Home Care, Hospice & Palliative Care is the trusted provider and expert resource
in delivering patient-centered care and services for patients and families needing
rehabilitation, advanced-illness or end-of-life care. With headquarters located
in St. Paul, Minn., services are provided in 19 Minnesota counties, 24 hours a day,
seven days a week.
Delivery of complex patient care autonomously. Most communication done by phone.
Delivery of quality patient care within the confines of fiscal and regulatory demands.
Education/Experience: High school diploma or GED equivalent required. AA degree
in Physical Therapy from an accredited program. Credentials to practice physical
therapy in the State of MN. 2-3 years of experience as a PTA, preferable in a home
care or an acute care setting. CPR certification every 2 years. BLS certification.
Skills: Strong observation skills of clients general medical condition, not just
PT needs. Good verbal and written communication skills. Ability to function well
in an independent and unstructured setting as an interdisciplinary team member.
Must be adaptable and flexible, willing to cross over geographic areas in time of
need. Excellent time management and organizational skills. Basic computer skills.
Understanding of and sensitivity to end of like issues and palliative care as it
applies to physical therapy goals.
Full time position (40 hours per week)
