Primary Function and Major Responsibilities
Provides leadership and direction to a multidisciplinary care team. Responsible for directing an efficient and effective model of care delivery and an outcome focused patient care model. Partners with the Home Care Executive to provide leadership and direction to Home Care staff ensuring quality cost effective care. Serves as a key member of the Home Care leadership team and participates in the overall strategic direction and operational performance of Home Care. Collaborates with all departments to make decisions appropriate to support the mission of HealthEast Home Care.
Responsible for the clinical and financial operation of HealthEast Home Care
Collaborates with the Director of Performance Improvement to integrate ongoing performance improvement into clinical and financial operations, as well as supporting a model for ongoing education of staff and evolution of practices and efficiencies.
Responsible for the performance of designated departments, meeting or exceeding budgeted objectives. Departmental productivity measures, survey status, clinical outcome scores, and performance to budget are included.
Establishes a collaborative process with the CFO/Director of Finance in the analysis and
planning of the agency's budget, as well as review of the agency's ongoing Income Statement/Profit & Loss statements.
Promotes a positive culture of accountability and competency in meeting established agency goals of providing quality patient care through appropriate utilization of staff. Develops situation leadership skills in front line management of staff. Provides oversight to the Clinical Managers responsible for the management and supervision of clinical teams of RN Case Managers, RN/LPN Visits Nurses,Physical Therapists, Occupational Therapists, Physical Therapy Assistants, Medical Social Workers and Home Health Aides to ensure effective and appropriate home care. Provides oversight of the scheduling/staffing of professional/non-professional staff assigned to the home health program.
Creates an environment of accountability as evidenced by performance of all team members whose work: demonstrates productivity within expectations; demonstrates OASIS competency; demonstrates clinical competency; elicits positive patient, employee and referral source satisfaction; demonstrates effective care planning and utilization management; and benchmarks favorably for attrition
Evaluates staffing needs based on: statistical management reports including but not limited to current caseload, productivity reports, growth projections, distribution of disciplines provided, budgeted FTE capacity; and collaboration with director of Referral Management & Community Relations,with input from the Care Manager.
Evaluates need for services and brings related information to management staff meetings for review and consideration.
Provides direction, guidance and support to Clinical Managers and Therapy Coordinator through activities including but not limited to: attending each team meeting at least monthly; conducting monthly clinical management meetings for review of performance based dashboard indicators (such as productivity, case mix weight, outcome scores, etc.) and other pertinent department information; conducting individual performance evaluations for each direct report position based on measured performance standards; and holding each accountable for defined performance standards.
Communicates with the Home Care Executive on issues of concern related to the operation of the agency.
Attends regularly scheduled management meetings to review agency outcomes: patient care and fiscal.
Participates in ongoing education presented by regulatory bodies, membership organizations, or related organizations through which regulatory or industry trends are communicated.
Supports HealthEast Home Care's mission.
Provides the opportunity for input from staff in the development and revision of policies and procedures.
Coordinates with management in the standardization of reports to be reviewed.
Keeps current with industry trends.
Participates in the organization's annual agency evaluation.
Works with the Director of Quality & Process Improvement, Intake Staff, Clinical Managers, Care Manager/Nurses/other staff members in patient care planning and conflict resolution.
Assists Clinical Management in the interpretation of: agency policies and procedures; nursing standards of practice; CMS regulations; Minnesota State Regulations; reimbursement issues; and accreditation guidelines.
Participates in budgetary planning and implementation.
Interviews and hires with Home Care Executive approval for positions directly supervised, which include: Clinical Managers and Therapy Coordinator.
Holds managers accountable to established agency standards and benchmarks.
Maintains confidentiality in all aspects of the job.
Performs other duties as assigned.
Qualifications
Education:
BSN or BA in Nursing, PHN. Masters of Nursing, Public Health or Business Administration preferred.
Experience:
Home Care nursing experience with a minimum of five years experience in a leadership capacity in Home Care and/or Hospice.
Special Knowledge, Skills and Abilities:
Excellent communication and interpersonal skills;
Working knowledge of home care federal/state regulations;
Knowledge of reimbursement sources within home care industry;
Maintain current knowledge of regulations and practices that govern work direction of a specific service line;
Must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patient's needs in relation to age and to provide the care needed as described in the department's policies and procedures.
