Responsibilities

  • Utilizing critical thinking while applying a bio-psycho-social model as well as knowledge of family systems theories, the medical social worker assists patients and their families with issues related to continuity of care
  • The Social Worker: Provides social work services including assessment, planning and intervention. Patients are identified and referred from providers, staff or community caregivers
  • Provides patients with education, advocacy, and decision making support related to the impact of medical illness, social needs, functional needs, or financial needs on healing, recovery chronic care, or end of life care
  • Assures appropriate reporting of vulnerability or suspected abuse as mandated by law

Education

  • 4-6 years post high school education with BSW or MSW - Bachelor or Masters Degree in social work required

Qualifications

Experience

  • Medical social work - minimum 1 year
  • Group facilitation - 2 years

Skills

  • Ability to work independently in ambiguous and complex situations with excellent problem identification, problem solving and work prioritization skills
  • Knowledge of psychosocial aspects of age and population specific human development particularly as it relates to illness and coping
  • Ability to plan and facilitate groups effectively
  • Effective communication and human relations skills
  • Knowledge and skills related to community resources and complex internal and external health care delivery systems
  • Ability to conduct interviews and complete psychosocial assessments within the context of the patient's environment in a condensed time frame
  • Demonstrated strength in evaluating and managing complex situations including conflict and decision management
  • Ability to function effectively as part of an interdisciplinary team
  • Knowledge of county, state and federal regulations

Licensure/Certifications

  • License Required: Social Work Licensure in State of Minnesota

Pay,Benefits, & Work Schedule

  • Part time position 28 hours per week
  • Day Shift

The city of Coon Rapids is a fast growing community on the Mississippi River just 20 minutes north of Minneapolis-St. Paul, and the metropolitan area gateway to the northern part of the state. Coon Rapids offers excellent access to highways, dynamic community shopping centers with major retail anchors, and a wide variety of recreational activities including neighborhood and regional parks, a municipal golf course, indoor ice arenas, hiking and biking trails and swimming pools.
