Responsibilities: To autonomously assess, prioritize, perform, document and evaluate professional nursing practice in regards to Diabetes Education at Regions Hospital; to perform and document diabetes education; to provide a safe and therapeutic environment; to coordinate, communicate, interpret, and explain all aspects of diabetes self-management to the patient, his/her family, health professionals and important others; to make appropriate referrals to interdisciplinary services to meet all the patients needs. Ability to initiate evidence-based practice changes in the care and management diabetic education patients and independently manage the Diabetes Education Business plan.

Qualifications: Licensed as a registered nurse by the State of Minnesota.  Graduation from an accredited school of nursing. Completion of a Bachelor’s Degree in Nursing or related field by year-end 2010 required.

Diabetes education experience required. Certification as a Diabetic Educator (CDE) within 1 year of hire required. Recent experience in acute care strongly preferred. Competency in Basic Life Support for the Health Care Provider required.
