Nurse (RN) Clinical Manager for
ESRD Care Management Program

Primary Responsibilities:

• Supports the Director of Clinical Operations in the development, implementation, monitoring and oversight of national partnership with a leading Dialysis Company

• Responsible for the day to day operations of a renal care management program including: creating and implementing staff training materials; managing work direction and performance of professional staff; managing ESRD patients as needed; and developing, monitoring and presenting to Executive Management Team quality metrics and standard outcome reports

• Conducts clinical quality audits and develops plans, programs and tools to support continuous improvement

Qualifications:

• Bachelor’s degree in Nursing (current MN RN license)

• 5+ years experience in chronic care of dialysis patients

• 2+ years experience in a supervisory/management clinical role

• Proficiency in software applications such as: Microsoft Word, Microsoft Excel, Microsoft PowerPoint

• Strong analytical, time management and problem solving skills

• Excellent verbal and written communication skills

• Prior Care Management/Disease Management experience preferred but not required

Our Chanhassen office is conveniently located off Highways 7 and 5. Positions are full-time.
