Nurse Case Manager

The Case Manager is responsible to perform services encompassed in the essential activities of case management, which are, assessment, planning, implementation, coordination, monitoring, and evaluation for assigned geriatric population.  Direct client contact would include activities in the areas of coordination and service delivery, physical and psychological factors, benefit systems and cost benefit analysis, case management concepts, and community resources.  The Case Manage provides services across a continuum of care that addresses the ongoing needs of the individual being served by the case management process.  These services are provided in an environment that involves multiple settings, interact with all relevant component of the client's health care system, and deal with the individual's broad spectrum of needs.
Qualifications

 
Required:  Registered Nurse in Minnesota  Bachelor's Degree or equivalent - (Associate Degree with two years nursing experience).  2 years of clinical nursing experience.
 
Preferred:  3 - 5 years case management experience encompassing work with all member age ranges and multiple Payors.   Utilization and quality management experience.  2 years demonstrated proficiency in computer software and hardware applications and basic knowledge of statistics.  Certified Case Manager.  LISW/MSW with Certified Case Manager.
