Rehabilitation Liaison / Intake Coordinator

Performs activities and duties related to recruitment, preadmission screening, education and admission of patient to inpatient acute rehabilitation and transitional care. Identifies and assesses potential patients, educates patients, family members and referral sources.  Serves as onsite liaison with both internal and external case management teams.
Qualifications
RN- BSN completed or RN - Associate degree with active process and clear plan for completion of
Bachelor's degree within 3 years, or Master's Degree in PT, OT, SLP or Rehab related area of healthcare.
Exceptional communication, interpersonal, marketing and customer service skills.  Three or more years of experience in discharge planning. Knowledge of regulation and requirements in post acute care settings including inpatient acute rehabilitation. Broad-based clinical knowledge preferred. Strong patient assessment skills.
