PREADMISSION SCREENING COORDINATOR Position DescriptionThis position is responsible to ensure members entering a skilled nursing facility meet appropriate level of care by completing OBRA Level I. Responsible to ensure that admission and retention of people with serious and persistent mental illness in nursing and boarding care facilities are referred appropriately. This position facilitates communication for MSHO and MSC+ members by collaborating with skilled nursing facilities and senior linkage line. Support Care Managers in clinical duties to ensure efficient work flow. Specific duties and responsibilities include: Determine appropriate level of care for members by completing the OBRA and comparing/reviewing results with the PAS. Complete HRA as appropriate and enters into state MMIS system. Facilitate a Level II screening through the county of member's residence as appropriate. Collaborate with skilled nursing facilities to verify admission of member and send PAS.Ensure timely delivery of: PAS, OBRA and entry of assessments into MMIS to comply with contractual obligations. Manage PAS inbox by reviewing and responding to all emails within one business day. Provide back up to care managers or associate care managers as license allows.Other projects and duties as assigned. EducationB.A. in social work. B.S.N., Registered Nurse with a nursing diploma (3 year program), associate degree in nursing or LPN. Current Minnesota license as an RN, LPN or social worker is required. Licensure requirements may depend on assigned product(s).Required ExperienceFamiliarity with medical terminology and the ability to interpret health information.Preferred ExperienceStrong customer service skills. Experience working for an HMO serving members with Medicare and/or Medicaid coverage. Experience working with state and federally funded programs. Experience working with culturally diverse populations, underserved populations, or the frail elderly.
