Mental Health Practitioner / Case Manager Responsibilities
The Practitioner is viewed as a primary agent of hope and change, and is required to work in full partnership with the client and his/her recovery process. The practitioner has a two focused level of responsibility: first, for the care, welfare, treatment, and safety of the clients as individuals; and secondly, for the facilitation of a work and a treatment culture that promotes trans-disciplinary team work, professionalism, cultural competency, and adherence to best practice principles and interventions. The Practitioner has a responsibility for client specific and program outcomes. Primary duties include assessment, crisis assessment and management, planning, individual counseling, teaching skills, group work, case management, household management, financial eligibility assistance, and on-call

Sub-Specialties

Housing Specialist: The housing specialist has skills and knowledge of housing, subsidies, and resource acquisition. This person tracks subsidy lists and availability and takes the lead on making sure team members are actively having clients apply for available subsidies. This person works on building a resource list and developing relationships with landlords and apartment managers. This person has knowledge and skills working with multiple layers of housing that are available to persons served.

Substance Abuse Specialist (LADC): The Substance Abuse Specialist takes the lead in screening, assessment, planning, treatment, and monitoring of progress, stages of change, and readiness. This person also provides training and consultation to the team's staff to help them gain competency in substance abuse and its treatment, using the concepts and methods of Integrated Dual Disorder Treatment. This role requires competency in IDDT, Motivational Interviewing, Harm Reduction, Cognitive Behavioral approaches, Relapse Prevention, and ability to provide Rule 25 assessments. Treatment would be applied based on client's stage of change readiness. This person plays a key role in the planning for clients with dual disorders. This person models skills and consultation; provides cross training to other staff members; attends all team meetings; and attends treatment planning meetings for clients with dual disorders.

Vocational/Supported Employment Specialist: The Supported Employment Specialist has experience providing employment services; job development, job coaching, and supported employment. The core services provided are engagement, completing vocational assessments, job development, job placement and coaching/or follow along services once employed an in school. The Supported Employment Specialist has skills and knowledge of benefit assistance, disability benefits, and economic assistance impacts. This specialist is involved in the treatment planning for clients who want to work or are working. They cross train other team members to develop support employment approaches. This person attends all team meetings.

Forensic Specialist: The Forensic Specialist must have experience working with the criminal justice system, jails, probation officers, attorneys, and the courts. They must be familiar with the unique and complex challenges that persons face when involved with the criminal justice system and have a mental illness. This person would collaborate and partner with the criminal justice system: courts, probation officers, attorneys, jail, and hospital, to improve mental health outcomes and reduce overall involvement with the criminal justice system. The Forensic Specialist would partner with the person served and Probation Officer in planning for treatment services

  • Conducts self in a professional and ethical manner in all work-related functions
  • Demonstrates productive individual and teamwork behaviors
  • Conducts overall rehabilitation goal planning, which may include living, learning, working, social, and spiritual environments
  • Conducts assessments: mental health status review, suicidal, vulnerable adult & immediate needs, functional, living skills, resource, chemical dependency, and vocational/employment
  • Interprets and summarizes findings with recommended treatment/rehabilitative activities and goals
  • Develops the individual treatment/rehabilitative plan; writes plan with differential goals and objectives
  • Monitors the individual treatment/rehabilitative process
  • Implements the treatment/rehabilitative activities and strategies
  • Prevents crisis at the lowest level of escalation
  • Manages and oversees client financial needs, living arrangement services, and monitors medication
  • Writes progress notes, summaries, and discharges
  • Facilitates transition and recovery support services
  • Provides staff and client consultation when on-call
  • Facilitates and monitors the civil commitment process
  • Achieves client specific and program outcomes
  • Adheres to Data Privacy Practices/HIPAA
  • Documents essential activities related to licensure, welfare, and safety
  • Monitors and records medication administration activities
  • Provides objective verbal and written critical incident analysis
  • Increases knowledge and skills based on client and program need
  • Completes specialized training for sub-specialty

Specific Requirements

  • Bachelor's degree in one of the behavioral sciences or related fields from an accredited college or university
  • At least 4,000 hours of supervised experience in the delivery of services to persons with mental illness
  • OR holds a graduate degree in one of the behavioral sciences or related fields and has at least 2,000 past BA hours of supervised experience in the delivery of services to persons with mental illness
