General Case Manager (ACT Team)

ResCare Minnesota's Mental Health Division has been providing psychiatric rehabilitation and person-centered care to adults whose lives are impacted by mental illness for over 30 years.

We are currently hiring for the following position: General Case Manager

The Case Manager must have experience providing case management services, work as part of a multi-disciplinary team, providing assessment, planning and prevention, skills teaching and group facilitation. Must have skills and knowledge of Evidenced Based Practices supported within Assertive Community Treatment.

Some responsibilities for all case managers and specialists include:

  • Case Management activities
  • Assessment, treatment planning, and progress summaries
  • Civil court commitment experience
  • Crisis assessment and prevention
  • Skills teaching and coaching
  • Supportive Counseling
  • Other tasks as assigned

Qualifications:

  • A Bachelor's or Master's degree in one of the Behavioral sciences from an accredited college or university
  • For a BA/BS, at least 2 years of experience post degree providing services to person's with a serious and persistent mental illness.
  • For MA/MS, at least 1 year of experience post degree providing services to person's with a serious and persistent mental illness.
  • Valid driver's license

Mostly day hours, may require weekend and on-call availability as well
