HEALTH EDUCATOR Position DescriptionThis position is responsible to develop, implement and lead health promotion programming for assigned UCare member programs. Participate in other member health initiatives. Provide expertise to other UCare departments and initiatives as assigned.·Coordinate UCare's MOMS (Management of Maternal Services) program. Serve as primary contact with Pregnancy Advisor vendor, doulas, and prenatal education providers, oversee contracts, reporting and evaluation. Promote breast-feeding and breast pump benefit. Coordinate childbirth education benefit. Coordinate provider pregnancy notification pilot. Maintain in collaboration with Provider Relations & Contracting and Clinical services. Moms and Parents resources.·Identify, screen and provide outreach to members that may benefit from the pregnancy advisor nurse program. Take necessary steps to enroll member.·Provide expertise to key internal and external customers regarding Health Promotion programs and services as assigned.·Identify potential new prevention initiatives and health promotion programs designed to maintain a leadership position in driving member responsibility for their health care.·Support development of Health Promotion department strategies and reports. Maintain current, accessible and useful methods of documenting and evaluating programs. Compile and prepare monthly reports representing activities, productivity, cost savings and recommendations.·Participate in projects or workgroups to assist UCare HEDIS Ratings. Manage UCare efforts in preventative health initiatives; including behavior change and preventative quality health improvement goals. Provide ongoing monitoring, evaluation and issue resolution of assigned projects. Develop key objectives, activities, outcome and process measurement plans. Manage within budget and deliverable dates. ·Collaborate with public health and community organizations on public health and community initiatives.·Other projects and duties as assigned.EducationBachelor's Degree in Health Education, Public Health or equivalent degree required.Required ExperienceExperience with maternal health, child and teen services and car seat education. Experience in a health delivery setting, such as health department, physician group, nonprofit agency, health plan or community clinic including experience in administrative or project management.Preferred ExperienceOne year experience working with diverse populations such as Somali, Hmong, Latino, or African American. Health prevention program development experience preferred.
