ASTHMA EDUCATOR (PART-TIME .5 FTE)

 

Position Description

This position is responsible to provide case management and education of members with asthma via telephonic follow up. This position also assists to improve HEDIS rates, improve health outcomes and support NCQA accreditation.  Specific duties and responsibilities include:

Provide outbound telephonic outreach to members who meet criteria for the asthma program.

Provide ongoing telephonic case management and assessments of member self-monitoring and medication adherence.

Assist with daily reporting to identify members eligible for the asthma program.

Provide telephonic follow up to members requesting to speak with an asthma educator.

Communicate with member care team, as needed.

Implement strategies to reach and maintain DM program goals, improve HEDIS rates and support NCQA accreditation. 

Provide outbound telephonic engagement calls to members with identified gaps in care to assist in obtaining preventive care and services.

Collaborate with members and/or family member, primary care physicians, clinic staff,

providers, and other relevant agencies to assure appropriateness of service that meets member needs and ensures desired outcomes.

Enter member information in the CareEnhance Clinical Management (CCMS) software.

 

Complete accurate, thorough and timely required documentation.

Serve as a resource to staff, providers and members enrolled in UCare administered DM programs. Provide ongoing education and information about DM programs as requested.

Maintain good working relationships and open communication with internal and external customers.

Other projects and duties as assigned.

Education

Registered Nurse or Respiratory Therapist. Current unrestricted RN or RT license in MN.

Required Experience

Two years' experience in asthma education, disease management or chronic condition care.

Experience working with individuals who have chronic conditions that foster self-management.

Preferred Experience

Certified Asthma Educator (AE-C) by national Asthma Educator Certification Board.  Managed care experience with a focus on case management of members with co-morbid conditions. Experience working with multi-cultural and variable socioeconomic populations highly desired.
