Position Description

This position is responsible to configure monthly MSHO/MSC + enrollment report and provide coverage support for Associate Care Managers (ACM).  Research and configure monthly enrollment report through data analytics; identify member clinic affiliations, investigate MMIS entries and attach information into CCMS.  Collaborate with counties and care systems to ensure accuracy in assignment of cases and clinics; including coordinating and responding to clinic change requests for MSHO and MSC+ members.   This position is also responsible to manage the Care Management Intake Line; telephone, fax, electronic communication and distribution of referrals for clinical services.  Specific duties and responsibilities include:


Configure enrollments report through complex data analytics. Search MMIS and attach most recent MMIS entry and other appropriate documents in CCMS for each member. Print a demographic page for care management assignment. 

Communicate new and transferred members to care systems and counties. Assist Care Systems and Counties to resolve enrollment roster issues.  Collaborate with Membership Billing & Enrollment to ensure enrollment issues are resolved and communicate issues and corrections to appropriate delegates.

Generate and send reports for members who experience a product or clinic change, coverage will terminate or have transferred out to another care system for case management to appropriate Care Manager for processing. 

Identify location of residence for newly enrolled members living in long term care facilities and verify clinic/provider affiliation; request clinic change if necessary.  Change status to “open case to assigned care manager”.   

Serve as a primary resource for enrollment and clinic questions for the MSHO and MSC+ team.  Forward UTF/transfer forms to delegates and counties. Attach relevant information in member files, distribute referrals for care management, and communicate urgent care coordination issues to the supervisor which includes checking Macess queues.  

Return and manage calls, faxes, and emails coming into the Intake Line within one business day. Manage the specialty Intake Inbox by forwarding referrals to Connect, UFS, and PMAP inboxes for case management. 

Coordinate PCC changes from delegates into the Macess Case Management PCC Change Queue, by determining the correct delegate and sending updated information to Enrollment for necessary changes.  After changes are finalized by Enrollment, communicate with delegate regarding the change. Ensure timely delivery of welcome letters and change in care coordinator letters to members. Check the MACESS Case Manager Waiver Authorizations Queue and forward SF's to the appropriate team within Clinical Services for correction.

Assist ACM's with entering LTCs into MMIS, processing DTRs, ordering DME for members, putting in authorizations for waivered services, scheduling member's doctor appointments, mailing out care plans and PCA assessments to members, faxing care plans and PCA notifications to providers, and acting as an interpreter on member calls or face to face assessments with care managers as time allows. 

Other projects and duties as assigned.


Associates degree or Bachelor's degree with focus on health, social service, psychology; demonstrated experience may be considered in lieu of degree. 

Required Experience

Three years of experience in a community health organization.  Demonstrated use and understanding of medical terminology.  Intermediate experience with databases such as Excel.

Preferred Experience

Experience working with state and federally funded programs.  Experience working for a health maintenance organization (HMO) serving members with Medicare and/or Medicaid coverage. Experience in working with culturally diverse populations, underserved populations or the frail elderly.
