Mental Health Practitioner
Primary Job description: Assisting the Bill Kelly House nurse with admissions and discharges of 16 SPMI clients, medication management and other tasks that include:
-counting and ordering medications from the pharmacy for clients twice a week.
-setting up evening and night medications and dispensing medications to clients.
-coordinating with the medical providers and the pharmacy to order and deliver medications.
-assisting clients with education on side effects of medications, purpose of medication, etc.
Other job duties include:
-Attend and participate in staff meetings weekly.
-Communicate pertinent information through verbal report and written documentation amongst staff members and shift changes.
-Encourage and monitor treatment plan expectations and program compliance.
-Conduct hourly inspections of the facility and engage in social interactions with clients.
-Transport clients to and from scheduled daily social, reactional activities.
-Assist with dinner and snack preparation/light housekeeping and clean up as needed.
-Assist clients in short term problem solving as needed.
-Co-facilitate the weekly house meeting once per week.
-Coordinate medical needs of clients with Bill Kelly House nurse.
-Assess medical needs of residents on a non-routine basis and utilize community service providers as needed.
-Provide crisis intervention and first aid as needed in response to resident problems.
-Coordinate with outside emergency services as needed.
-Complete necessary paperwork: i.e resident incident reports, referrals, etc.
**Upon accepting this position, it is requested that the first week of employment you can be flexible with your schedule so that you can work one on one with the nurse and become medication trained. This includes one day working 6:30am -2:30pm to become medication trained. The other 4 days can be 10am-6pm, these times can be negotiated with the nurse.
