JOB SUMMARY: The primary purpose of this role is to educate patients, staff, and public regarding the management of diabetes as well as healthy eating and wellness opportunities.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Educate patients, families, and appropriate support systems about healthy eating, healthy weight, diabetes self-management and related issues including:
  • Pathophysiology of diabetes mellitus
  • Nutrition, diet, exercise, pharmacologic interventions, and activity
  • Glycemic control monitoring
  • Prevention and management of acute and chronic complications
  • Psychosocial adjustment, problem-solving skills, and stress management
  • Use of the health care delivery system
  • Use established principles of teaching-learning theory and life-style counseling
  • Assess educational needs of patients and families
  • Develop teaching-learning processes for patients and families
  • Implement individualized educational plans and evaluate outcome criteria
  • Document patient process and progress
  • Develop written educational plans based on components of the educational process
  • Coordinate plan among health team members
  • Provide individualized education based on a progression from basic survival skills to advanced information for daily self-management

SUPPLEMENTAL DUTIES AND RESPONSIBILITIES:

  • Maintain a positive and respectful attitude while supporting the clinic vision
  • Maintain required certification and licensure applicable to degree
  • Maintain a neat and well groomed professional appearance
  • Interact positively with all interdisciplinary team members
  • Maintain confidentiality of all information acquired through clinic employment
  • Establish and maintain positive working relationships with patients, providers, internal personnel, and outside facilities
  • Follow clinic emergency/safety protocols and attend mandatory meetings
  • Abide by clinic protocols, ergonomic recommendations, and OSHA standards
  • Adhere to the philosophy and provide comprehensive care according to a patient centered healthcare clinic

SKILLS AND KNOWLEDGE:

EDUCATION: Minimum: Registered Dietitian or Registered Nurse licensed in

the State of Wisconsin with appropriate training

Desired: Masters degree and certified by the American Association of

Diabetes Educators

EXPERIENCE: Minimum: 2-5 years in a clinical setting

Desired: 5+ years in a clinical setting
