HEALTH PROMOTION SPECIALIST - PERINATAL OUTREACH

Position Description

This position is responsible for providing telephonic support to UCare's State Public Programs members during and after pregnancy.  The Perinatal Outreach Specialist will assist members with preventive, prenatal and postpartum care access, identify members with high risk pregnancies and connect members with appropriate support services and resources. 

·Provide telephonic assessment of pregnant member. Outreach will occur during pregnancy and postpartum.

·Complete risk assessment of pregnant members. Identify high risk members for additional support and resources, including referrals to internal UCare resources for member support, UCare partners and external resources for a healthy pregnancy and baby.

·Provide ethnically and culturally appropriate care and support.

·Document case notes and report all relevant member information in the appropriate internal systems at UCare. Documentation must be timely and accurate.

·Collaborate with the Health Promotion Health Educator to continuously improve member identification, assessment and outreach.

·Meet and maintain all established performance metrics.

·Assist in efforts to improve HEDIS rates and the overall health outcomes of pregnant members

·Support the additional work of the Health Promotion team as needed.

·Other projects and duties as assigned.


Education

Post-secondary degree in health education, health promotion, nursing, or other health related field. Current unrestricted certification/license in specific field, if applicable.

Required Experience

Two years of experience with perinatal care coordination or case management in a health care setting (hospital, clinic, home care, etc).

Preferred Experience

LPN or Registered Nurse preferred with current Minnesota license.  Experience working with state and federally funded programs.  Experience working for a health maintenance organization (HMO) serving members with Medicaid coverage preferred. Experience in working with culturally diverse and underserved populations. Strong customer service skills.
