MEMBER ENGAGEMENT SPECIALIST Position DescriptionThis position is responsible to provide outbound telephonic engagement calls to engage members in an effort to improve HEDIS rates and disease management program participation. This position will focus on recruiting and enrolling members into health coaching/disease management programs. Specific duties and responsibilities include: Provide outbound telephonic engagement calls to members who meet criteria for disease management programs.Perform daily administrative duties to identify members eligible for disease management programs. Collaborate with teams to document, schedule and identify project timelines, milestones, resource allocation and problem-solving strategies. Complete referrals to the Disease Management (DM) team if a member opts to participate in a DM program. Add members who wish to opt-out of further calls/mailings, etc. into the “Opt-Out” list.Process daily outreach lists, referrals and inbound disease management phone callsCollaborate with Disease Management Manager and Health Care Economics (HCE) to update member contact lists. Provide outbound telephonic engagement calls to members with identified gaps in care to assist in obtaining preventive care and services.Perform point of contact duties between member and delegate partners as necessary.Identify and implement strategies to reach and maintain DM program goals, improve HEDIS rates and support NCQA accreditation. Complete productivity tracking as directed.Collaborate with members and/or family member, primary care physicians, clinic staff, providers, and other relevant agencies to assure appropriateness of service that meets member needs and ensures desired outcomes.Enter member information in the CareEnhance Clinical Management (CCMS) software. Complete accurate, thorough and timely required documentation. Support and maintain enrollment in CMS chronic condition improvement program.Maintain good working relationships and open communication with internal and external customers.Other projects and duties as assigned. EducationTwo years of post-high school at an accredited college, university or vocational school with a focus on health, social service, psychology or related coursework. Required ExperienceTwo years of customer service or sales and marketing experience in a call center, health insurance, physician group practice, or community agency. Proficient PC skills and in Microsoft Office (with emphasis on Excel spreadsheets).Preferred ExperienceKnowledge of and experience in telephonic outreach coordination.
