HEALTH PROMOTION OUTREACH SPECIALIST Position DescriptionThis position is responsible for providing telephonic outreach to members during and after pregnancy. Outreach will also include Child and Teen Checkup reminders. The Outreach Specialist will assist members with preventive, prenatal and postpartum care access, identify members with high risk pregnancies and connect members with appropriate support services and resources. Specific duties and responsibilities include:
- Provide telephonic outreach to; Pregnant members - Complete pregnancy risk assessment, identify high risk pregnancies, and provide additional support and resources to promote healthy pregnancies and birth outcomes (including referrals to internal resources for member support, UCare partners and other resources); Postpartum members to ensure timely follow up care; and Parents of UCare members to promote Child and Teen Checkup visits.Provide ethnic and culturally appropriate support. Document case notes and report relevant member information into appropriate internal systems. Documentation must be timely and accurate.Collaborate with the Health Promotion Health Educator to continuously improve member identification, assessment and outreach.Meet and maintain all established performance metrics.Assist in efforts to improve HEDIS rates and the overall health outcomes of pregnant members Support the additional work of the Health Promotion team, as needed.
EducationPost-secondary degree in health education, health promotion, or other health related field.
Required ExperienceTwo years of experience with direct member outreach and education.
Preferred ExperienceExperience working with state and federally funded programs. Experience working for a health maintenance organization (HMO) serving members with Medicaid coverage preferred. Experience in working with culturally diverse and underserved populations. Strong customer service skills.
