Assigned 75% to the Community Living Center and 25% to the Adult Day Health Care Program. Is responsible for providing social work services to Veterans. Is a member of an Interdisciplinary Treatment Team (ITT). Is primarily responsible for completing biopsychosocial history and assessments, completing psychosocial portions of care plans & Minimum Data Set (MDS). Is responsible for facilitating ITT meetings, emphasizing culture transformation, including resident centered care planning.


The incumbent is responsible for completing the following duties/responsibilities:
> Provide orientation to new residents of the CLC/ADHC, communicating with Veterans & family members/significant others the purpose of CLC, expected length of stay & goals for discharge on admission
> Conduct biopsychosocial assessments to determine needs of Veterans & family members
> Participate in the treatment planning process with other disciplines
> Facilitate the Long Term Care Co-Pay process with the Veteran & family members
> Participate in discharge planning with other disciplines, Veterans & family members. Will be responsible for ensuring discharge plans are executed timely and appropriate
> Provide consultation/education to Veterans & families on community resources & VA services/benefits
> Act as a consultant to other team/staff members
> Act as liaison between VA & community resources
> Provide case management to Veterans & families, as needed, throughout the continuum of care.
> Provide a full range of social work services within commonly accepted standards of social work practice
> Provide psychosocial treatment to assist Veterans & family members with psychosocial concerns
> Assist Veterans with crisis situations
> Serve as the CLC & ADHC contact person for family members/significant others. Will coordinate family conferences/meetings & address family issues that impact the Veteran's health/functioning
> Serve as an advocate for residents with facility staff & treatment team members
> Assist Veterans address advance care planning and end-of-life issues
> Provide wellness/prevention education & facilitate resident/family support groups as appropriate
> Supervise the practice of social work students, or a less experienced social worker as applicable
> Coordinate treatment & communicate with the care lines to facilitate continuity of care for veterans
> Participate in various Service Line/health care system workgroups, committees, and/or councils to represent and contribute social work expertise as assigned
> Maintain data & statistical compilations to comply with Joint Commission, VA and health care system policy and
> Document all social work interventions and activities in the resident's clinical record utilizing CPRS and ensure appropriate hand off when transferring patient care

Work Schedule: Administrative Days, Monday-Friday, 8:00 a.m. to 4:30 p.m
