Referral Case Manager/Community Utilization Management RN
This is a shared position to provide coverage for both the Referral Case Manager and Community Utilization Management nurse roles. The majority of the work will be in the Referral Case Manager position.
Referral Case Manager Role Overview: Facilitate the seamless transfer of veterans for inpatient or outpatient services from other healthcare facilities (VISN and Community) to the MVAHCS. This position also facilitates transfers, when medically appropriate, from the CBOC diverts. The RN is expected to contribute to other UM functions as assigned, including but not limited to: resource for UM community nurses and Traveling Veteran Coordinator, committee participation, improvement efforts, and collaboration with service lines as needed.
Community UM Nurse Role Overview: Has accountability for follow-up for in-patient veterans in the community hospitals. This will include but is not limited to regular calls to fee-basis inpatient facilities and the associated documentation/communication of updates. The RN is expected to contribute to other UM functions as assigned, including but not limited to: authorization for Fee Basis Clinical Tracking, cross coverage for Traveling Veteran Coordinator, Referral Case Management, committee participation, and improvement efforts
REQUIRED RN EXPERIENCE: 3-5 YEARS
EDUCATION: BSN REQUIRED, MA DEGREE PREFERRED
CONTINUOUS IMPROVEMENT/READINESS/QUALITY/DATA MANAGEMENT EXPERIENCE PREFERRED
Work Schedule: M-F 8:00-4:30
