Gastroenterology RN Case Manager
The GI Case Manager is a Registered Nurse who is an active member of the GI Team and works closely with Department staff to deliver efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. In this role, the GI Case Manager works in partnership with the GI/ GI Liver Core team to provide comprehensive care to the patient population seen within the GI Department including GI Liver Care and Transplant.
The GI Case Manager uses the nursing process and evidence-based practice in developing patient-driven holistic care plans for life. This includes collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. He/she will provide patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. This RN also assesses for alterations in patient's health status, implements and provides interventions based on disease management approved protocols and orders, with primary objectives of ER/Hospital avoidance, improved quality of life, improved clinical outcomes and care coordination.
Education/Experience: Bachelor's Degree in nursing preferred; 3 years or more of RN nursing experience is required. Case Management experience in an outpatient clinic setting is strongly preferred.
Work Schedule: Full-Time, primarily administrative days. However work hours may be variable to meet program needs.
