This position is eligible for a $1500 sign-on bonus.



Position Description

This position is responsible to provide case management and education of members with asthma via home visits and telephonic follow up.  This position also assists in efforts to improve HEDIS rates, improve health outcomes, and support NCQA accreditation.  Specific duties and responsibilities include:  

Provide in home assessments for potential asthma triggers, adherence with medications, inhaler techniques and perform an Asthma Control Test (ACT).

Provide ongoing telephonic asthma case management and assessments of member self-monitoring and medication adherence including program engagement calls. .

Provide outbound telephonic outreach to members who meet criteria for the asthma program and/or request to speak with a respiratory therapist.

Utilize motivational interviewing and health behavior change techniques during telephonic sessions with members.

Provide education to members as it relates to their chronic condition

Communicate regularly with members, providers, case managers and other health care team members also engaged in the members' care.

Accept and provide referrals to/from disease management, case management, hospitals, clinics and other available resources.

Perform daily administrative duties to identify members eligible for asthma programs.

Assist in the development of Disease Management (DM) educational materials used for case management purposes.

Implement strategies to reach and maintain DM program goals, improve HEDIS rates and support NCQA accreditation.

Enter member information in the Care Enhance Clinical Management (CCMS) clinical documentation system.  Documentation is accurate, thorough and timely.

Assist in the development and ongoing support of measurement systems to evaluate DM program outcomes and processes.  Assist the Disease Management Supervisor with data for the monthly report as requested.

Implement case management for members of various socioeconomic and cultural groups.

Serve as a resource to staff, providers and members enrolled in UCare administered DM programs.  Provide ongoing education and information about DM programs as requested.  

Other projects and duties as assigned. 


Respiratory Therapist.  Current unrestricted RT license in Minnesota.

Required Experience

Two years experience in respiratory therapy, disease management or chronic condition care.  Experience working with individuals who have chronic conditions that foster self-management.

Preferred Experience

Managed care experience with a focus on case management of members with co-morbid conditions.  Experience working with multi-cultural and variable socioeconomic populations highly desired.
