Position Description

This position is responsible to provide telephonic health coaching, management and education to members living with complex physical and behavioral health conditions.  Works directly with member to engage, assess and provide coaching to members living with complex health conditions. Refer as needed and collaborate with Care Manager and treatment providers.  Specific duties and responsibilities include:  

Provide individual telephonic health coaching and management to members with complex physical and behavioral health conditions.

Engage members in ongoing health coaching using motivational interviewing and health behavior change techniques during telephonic coaching sessions with members

Assist members in reaching personal health related goals.

Provide education to member as it relates to their chronic condition.

Evaluate and address social health needs by actively linking member to Care Manager. 

Identify and monitor member needs, including significant changes in condition which may warrant early intervention for medical problems.

Communicate regularly with member, providers, case managers and other health care team members also engaged in the members' care.

Enter member information in the Care Enhance Clinical Management (CCMS) clinical documentation system.  Documentation is accurate, thorough and timely.

Meet and maintain all established caseload and performance metrics.

Implement health coaching for members of various socioeconomic and cultural groups.

Collaborate with multidisciplinary staff to bring perspective based on your discipline and experience.

Other projects and duties as assigned. 


Bachelor's degree or higher in health education, health promotion, nursing, nutrition, social work or other health related field. Current unrestricted certification/license in specific field, if applicable.

Required Experience

Two years' experience in disease management or chronic condition management working with individuals with co-occurring physical and behavioral health conditions.  Experience with individual behavior change management and motivational interviewing. Experience working with individuals who have chronic conditions that foster self-management.

Preferred Experience

Managed care experience with a focus on health coaching individuals with co-occurring physical and behavioral health conditions. Knowledge of psychological and social aspects and characteristics of emotional disturbances, mental illness and addictive disorders. Experience working with multi-cultural and variable socioeconomic populations highly desired. Health coaching certification.
