Position Description

This position is responsible for developing and maintaining positive working relationships with delegate care systems, agencies and counties. The Clinical Care System Liaison also develops and delivers care system product training related to Clinical Services (CLS) for internal staff and external customers. This includes, but is not limited to, serving as a key contact for care system and county inquiries about UCare care coordination products and care coordination processes, and leading clinical training for UCare's care system and county partners.  In addition, this position has primary responsibility to ensure that relevant training materials are developed and maintained. Specific duties and responsibilities include:  

Develop and maintain a working knowledge of new and existing care system products relative to CLS functions, and introduce new care system products to CLS staff and external customers.

Serve as a key contact and provide support for care systems and counties for clinical issues as they relate to UCare products.

Represent CLS in internal and external care system and county-based work groups or initiatives, such as the care coordination work groups, etc.

Identify knowledge gaps with external customers, identify the need for training and develop and deliver the training curriculum.

Develop and maintain CLS-related training materials for external customers, including product information and updates, care coordination manuals, CLS newsletters, etc.

Conduct regularly scheduled and ad hoc CLS care coordination-related training for care systems, counties and internal care coordination staff.

Maintain a working knowledge of regulations, and team functions, policies and procedures.

Coordinate and lead care coordination-related projects and processes as directed, such as monthly enrollment files, transitioning large groups of members in response to product change, implementing a new product at county sites, responding to information requests by regulators, etc. Prepare and implement a project plan, statement of work, and/or process flow related to different projects, and prepare basic results analysis as needed.

Collaborate with Clinical Care System/County Liaisons and act as a back-up to support and maintain positive working relationships with care system and county staff.

Prepare monthly reports on activities and identify opportunities to increase efficiencies, productivity and quality of work.

Other projects and duties as assigned.   


Registered Nurse (RN), or Social Worker with current unrestricted license in Minnesota required.

Required Experience

Two years experience in training. Strong health plan, managed care, health services, or other related experience.

Preferred Experience

One year experience in Medicaid or Medicare risk managed care programs. Product knowledge related to MSHO/MSC+ and SNBC products. Experience presenting to groups.
