Position Description

This position is responsible to provide outreach to identified providers to verify information; manage and execute the UCare Disclosure of Ownership (DOO); and Provider Questionnaire Projects. This position will support efforts to update and validate provider information.

·Provide outbound telephonic verification calls to providers.

·Complete tracking documents as directed and within project timelines.

·Maintain good working relationships and open communication with internal and external customers.

·Coordinate the preparation and distribution of required materials to UCare providers via email and US Mail.

·Audit returned documents and conduct follow up with the provider as needed to ensure completeness.

·Complete data entry of form elements to master file. Manage project tracking file.

·Maintain provider information in contract tracking database to ensure data integrity.

·Upload completed documents.

·Other projects and duties as assigned.


Associate degree in a related field preferred; demonstrated experience may be considered in lieu of degree.

Required Experience

Customer service experience in a call center, health insurance, physician group practice, or community agency. Proficient PC skills and Microsoft Office (with emphasis on Excel spreadsheets).  Document management experience.

Preferred Experience

Knowledge of and experience in telephonic outreach coordination. Excellent computer word processing and database experience.
