Case Worker

Minnesota Mental Health Clinics is seeking a full-time employee for the Horizons Case Management Program. Horizons is a community support program providing a range of services to seriously and persistently mentally ill adults 18 and older in Dakota County. This position provides Targeted Case Management (TCM) services to adults in Dakota County who are experiencing mental illness. This individual will assist adults who are diagnosed with a serious and persistent mental illness to access needed mental health, medical, social, educational and vocational services to meet their individual mental health recovery needs. This position also requires outreach to clients in the community, effective engagement skills, positive attitude, strong accountability, and teamwork.

Qualified candidates will possess a BA/BS and two years of mental health experience, or MA/MS in human services and one year experience. Must have a valid Minnesota driver's license and good driving history. Competitive wages with good benefits and opportunity for bonuses.
