Job Details

The Advantage Services Roving/Supportive Housing Coordinator (SHC) provides Supportive Housing Services at CBC and fee-for-service Supportive Housing programs as well as roving services for residents of CommonBond Advantage Services Programs to any site experiencing a staffing shortage.

ESSENTIAL JOB FUNCTIONS AND ASSOCIATED TASKS

1. Coordinates Supportive Housing Services (SHC)
A. Serves as main liaison between Advantage Services staff and Property Management at multiple Supportive Housing sites.
2. Coordinates Advantage Services (Roving)
A. The Roving Advantage Services Coordinator is responsible for providing short-term services to Residents of CommonBond Advantage Services Programs, including youth programming.
B. CommonBond properties requiring a Roving Advantage Services Coordinator will be scheduled at properties in the Metro area as well as in Greater MN.
C. Responds to referrals for individualized services that includes ongoing assessments of community and individual needs.
3. Assists prospective residents and referral sources in navigating the housing application process (SHC)
A. Assists prospective residents in gathering of required eligibility and application documentation, including verifications of homelessness at Supportive Housing sites.
B. Assists in guiding prospective residents through the screening and application process in coordination with property management to assure fair housing requirements are met in every applicant case.
4. Provides comprehensive case management services for assigned individual residents (SHC)
A. Conducts a thorough strengths-based assessment with each new resident, and reassess at least every three months in conjunction with individual plan development.
B. Develops a goal with each resident and updates the plan with each resident on an ongoing basis.
C. Links residents with new community-based services as individual plans warrant, and assists residents to maintain existing community-based linkages as needed.
5. Assists residents to positively resolve situations that place tenancy at risk (Roving and SHC)
A. Meet with each resident (individual or group) within 30 days of move-in to assure resident understanding of lease, handbook, and house rules.
B. Responds to specific property management concerns regarding individual resident lease adherence within 24 hours of being informed of a concern (within 1 hour in emergency situations).
C. Works collaboratively with both residents and property management to fully resolve lease adherence concerns/housing risk and minimize lease terminations.

MINIMUM REQUIREMENTS
1. Associates or Bachelor’s degree in related field or at least three years’ experience in a related field.
2. Knowledge of local social service programming and resources
3. Two or more years of experience implementing human service programs focused on ethnically, socially and/or economically diverse populations.
4. Four or more years experience implementing programs that address mental health, chemical health, and/or co-occurring disorders and homelessness.
5. Four or more years experience implementing programs that use evidence-based models of intervention e.g. IMR, supported employment, supportive housing.
6. Excellent interpersonal, verbal, and written communication skills.
7. Demonstrated experience working within diverse communities, which include people of various ages, gender, sexual orientations, national origins, income levels, ability levels, religious affiliations, races, and languages.
8. Experience utilizing MS Office and web based programs.
9. Ability to respond to specific property management concerns regarding individual resident lease adherence within 24 hours of being informed of a concern (within one hour in emergency situations).
10. Be Certified in CPR and First Aid or have the ability to become certified within six months of starting position.
11. Be at least 21 years of age.
12. Provide proof of a current and valid driver’s license issued in the state of residency.
13. Have or be willing to obtain adequate automobile insurance.
14. Meet the company Motor Vehicle Record (MVR) criteria.
15. Have a vehicle.

PREFERRED REQUIREMENTS
1. Bachelor’s Degree in social work, human services or similarly related field.
2. Knowledge and experience with supportive housing funding streams including LTH and Section 8
3. Licensed Mental Health Professional
4. Residential and/or clinical experience with persons experiencing mental illness, chemical dependency, co-occurring disorders, and homelessness.
5. Experience with medical social work and documentation.
6. De-escalation and crisis intervention skills.
7. Knowledge and experience in using evidence-based, recovery-oriented tools and techniques and ability to train and support staff in learning.
