MEMBER ENGAGEMENT SPECIALIST (TEMPORARY)

Position Description

This position is responsible to provide outbound telephonic engagement calls to engage members in an effort to improve case management program participation. This position will focus on recruiting and enrolling members into case management programs.

*Provide outbound telephonic engagement calls to members who meet criteria for case management programs.

*Perform daily administrative duties to identify members eligible for case management programs.

*Collaborate with teams to document, schedule and identify project timelines, milestones, resource allocation and problem-solving strategies.

*Complete referrals to the case management team if member opts to participate in CM program.

Process daily outreach lists, referrals and inbound case management phone calls

*Collaborate with Case Management Supervisor and Health Care Economics (HCE) to update member contact lists.

*Provide outbound telephonic engagement calls to members with identified gaps in care to assist in obtaining preventive care and services.

*Identify and implement strategies to reach and maintain CM program goals.

Complete productivity tracking as directed

Collaborate with members and/or family members, providers, clinic staff, and other relevant agencies to assure appropriateness of services that meet member needs and ensures desired outcomes.

Enter member information in the clinical documentation software. Complete accurate, thorough and timely documentation.

Maintain good working relationships and open communication with internal and external customers

Other projects and duties as assigned.

Education

Two years of post-high school at an accredited college, university or vocational school with a focus on health, social service, psychology or related coursework.

Required Experience

Two years of customer service or sales and marketing experience in a call center, health insurance, physician group practice, or community agency. Proficient PC skills and in Microsoft Office (with emphasis on Excel spreadsheets).

Preferred Experience

Knowledge of and experience in telephonic outreach coordination. Excellent computer word processing and data base experience.
