Marshfield Medical Center - Eau Claire is a 44-bed, full-service hospital offering advanced care capabilities including cardiovascular surgery, advanced spine surgery, 24/7 emergency medicine, critical care, inpatient/outpatient oncology care, and more.

The RN Case Manager serves as part of the collaborative care team, addressing the patient's plan of care and expected outcomes with special attention being given to progression towards discharge. The RN Case Manager serves as a resource to the care team with regards to discharge planning, and provides review of medical utilization to third party payers to obtain certification for the hospital stay.

This position will work in the setting of Eau Claire - Marshfield Medical Center.

Requires:

• Diploma or degree in nursing required; Bachelor's degree in nursing preferred
• 3 years of experience in a medical facility required; Discharge Planning, Utilization Review or Case Management experience in a clinical/hospital setting preferred
• Current unrestricted State of Wisconsin Registered Nurse license
• Basic Life Support (BLS) certification awarded by the American Heart Association (AHA)
