Join our Home Health Agency as we expand our Skilled Nursing Department to include postpartum visits for moms and babies (no IBCLC certification required). In addition to well-baby visits with newborns, nurses will be joining our existing skilled nursing team for visits with our medically complex population. 

In this exciting new role you will be key in ensuring the highest level of patient care, growing our referral base as well as ensuring staff are well trained and competent!

Duties may include: 

• Manage staff, provide feedback, ongoing competency training
• Ensure the highest level of patient care and customer service
• Develop and cultivate relationships with external resources, stakeholders and partners to drive referrals
• Deliver performance feedback and work with staff on conflict resolution as needed
• Oversee department goals and performance
• Schedule is Monday-Friday with rotating on-call

Qualifications:

• Experience in skilled nursing visits
• Management experience a plus
• Lactation consulting a plus
• Associate’s degree in nursing and current MN RN license 
• Excellent communication and interpersonal skills 
• Competency in healthcare operations, case management, compliance, and care coordination

Preferred Qualifications: 

• BSN Education 
• Public Health Nurse
