Position Description

This position is responsible to address individuals' health needs at all points along the continuum of care, including in the community setting, by increasing participation, engagement and targeted interventions for a defined population. Maintain or improve the physical and psychological well-being of individuals and address health disparities through cost-effective and tailored health solutions. Contributes to the company's mission and vision by supporting, coordinating, developing and maintaining the workflows necessary to shift from a sole emphasis on the individual member to focusing on both the individual member and groups of members. Facilitate the oversight and direction for designing, implementing and supporting an organization-wide population health management program that is aligned with the Triple Aim.

• Oversee and manage project plans, timelines, programs, development of reports, policies related to PHM activities.

• Identify key PHM priorities and work on programs and interventions targeted to improve the health of all members, quality and total cost of care.

• Ensure UCare is compliant with regulatory requirements for population health. This includes an understanding of applicable State, Federal and NCQA requirements and the ability to keep updated with any new requirements and to modify projects, as needed.

• Collaborate and engage with departments throughout the organization to implement the population health program and promote a culture consistent with UCare's mission, vision and core values.

• Accountable to key PHM metrics including quality measures, utilization measures and financial measures. Oversees monitoring, measuring and reporting activities and collaborates with others to evaluate findings and improve process and outcomes.

• Build and support effective relationships with internal and external stakeholders and organizations.

• Review, prepare, analyze and present reports and recommendations regarding programs, services and other applicable areas in order to provide concise and accurate information that aids in decision-making.

• Maintain knowledge of industry trends and practices.

• Other projects and duties as assigned.


Bachelor's degree in business administration, health care, information systems, social sciences, public health or another related field required. Master's degree preferred.

Required Experience

Five years of experience in quality improvement and in health care delivery system and/or project management experience. Demonstrated process improvement and project management skills. Strong verbal and written communication skills. Problem-solving, decision-making, flexibility, disciplined, team management, business writing, excellent communication skills both written and verbal, able to work under pressure, deadline oriented and customer service skills.

Preferred Experience

Five years related experience. Experience in Population Health Management. Formal training in project management. Experience working in managed care organization.
